The first document we have is letter templates that can be used by you or your doctor while navigating various types of care.

Letter Templates

Doctor's Note for Workplace Misgendering Issues - This note provides a clear template to be used by a doctor to outline using your correct name and pronouns in a workplace as a medical directive.

Doctor's Note for Changing Sex Designation - This note provides a clear template to be used by a doctor to provide an effective note for someone submitting a form to legally change their sex/gender designation.

Doctor's Note for Surgery Referral - This note provides a clear template to be used by a doctor to provide an effective surgery referral for any gender-related surgery

Personal Note for Care Providers - This note is a template to provide to new clinics or care providers around how to address you and your body parts. You can request this note be appended to your file.

Trans Health Card - Designed by the Aids Committee of Toronto, this card can quickly summarize information regarding your transition to give to new or existing care providers to better understand your gender and transition without you needing to explain it all.

Gender affirming hormone treatments (GAHT) or hormone replacement therapy (HRT) is a common part of transitioning. Here we have collected some information regarding GAHT. Please note we are working on a document to discuss initiating HRT in Waterloo region and we will update that page with that document once ready.

In Waterloo Region, the Gender Variant Working Group (GVWG) hosts binder try- on events, where you can get help sizing and more information about binding in general. For more information, check their website:  https://wwgendervariant.wixsite.com/gvwg/about-1

SHORE is also a drop off location for the GVWG's binder donation program! If you have binders that you are no longer using, that are in good shape, you're welcome to drop them off to SHORE Centre at our 50 Westmount Rd N location, and we will bring them to the GVWG for distribution to others!

Pap Tests for Gender Diverse Folks

We do have some resources regarding pap tests for trans and gender diverse folks, however many are specific for clinicians, and training them how to perform an inclusive pap test!

Lastly, SHORE has always offered trans inclusive pap tests, so all of their clinicians are well versed and experienced with supporting patients of any gender with pap tests. If you would like to book an appointment for a test at SHORE, you can request one online at shore.inputhealth.com. If you'd like to meet with that clinician first to chat about the process, or additional supports, they can do that as well. You are also always welcome to bring a support person to your appointments at SHORE.
